The episode "The Great Fusilli", is the last episode of season 1. It ends with Courage controlling the dead bodies of Muriel ans Eustace as puppets in attempt to keep his life with them going. The more you think about the subject, the more psychological it gets. This scene implies that after season 1, all of the the tings happening are just thoughts Courage has in his mind. It implies that Muriel and Eustace truly died, but courage has gone insane and made himself and us think that they are still alive.

All the kids on this show are actually dead. Each of them came from a different time period in American history.
Rolf, the weird kid with a European accent died in the early 1900s in a farming accident.
Johnny, the one who is best friends with a plank comes from the 1920s when having a piece of wood with a face painted on it made you the coolest kid on the block.
